This blog is a way of sharing the information and resources that have helped me to recover my son Roo from an Autism Spectrum Disorder. What I have learned is to view our symptoms as the results of underlying biological cause, which can be identified and healed. I say "our symptoms" because I also have a neuro-immune disorder called Myalgic Encephalomyelitis.

And, of course, I am not a doctor (although I have been known to impersonate one while doing imaginative play with my son)- this is just our story and information that has been helpful or interesting to us. I hope it is helpful and interesting to you!

Dietary Fats and Oils

Sources of oils and fats

Microbes in intestines convert fiber into short-chain fatty acids.  This should be vegetable fiber, NOT grain fiber or capsules/drink.  

Salmon Roe - richest source of omega-3 fatty acids (EPA and DHA), contains 1800 of EFAs in one ounce of salmon roe.  Excellent support for the neurological system.  Also high in other nutrients as well.

Salmon Roe – Unsurpassed for Nourishing the Brain

Speak - This is a supplement that is a combination of oils that supports brain function, especially in regards to speech.

Specific Oils and Fats

Black Seed Oil is a mast cell stabilizer.

Chia Seed - contains omega-3 fatty acids and other essential fatty acids including alpha-linolenic and linoleic acid, oleic acid, and palmitic acid.

CLA (Conjugated Linoleic Acid) found in red meat and grassfed dairy, has anti-tumor properties.  

Coconut Oil contains Lauric Acid, an antiviral, and Caprylic Acid, which is antifungal.  

Cod Liver Oil (CLO) Source of omega-3 fatty acids oils as well as vitamin A and vitamin D.  Omega-3 fatty acids increase brain-derived neurotrophic factor (BDNF) which supports growth of new neurons.  They are also anti-inflammatory, especially in the brain. 

Lard is a natural source of vitamin D and choline and contains some vitamin E.  Avoid lard that has been hydrogenated.  

Olive Oil is an excellent source of omega-9 fatty acids, including oleic acid.  It has the highest proportion of monounsaturated fatty acids of all of the liquid vegetable oils.  It contains 13% of RDA of vitamin E and about 10% of RDA of vitamin K.  The extra-virgin variety has a higher mount of polyphenols, which are natural antioxidants.  

What Does Research Actually Say About Healthy Fats? 

Is Saturated Fat Bad For You?
Ketogenic diet therapy doesn't have to include any saturated fat; many people eat a vegan keto diet or one that is vegetarian without dairy, or some other specialized form of keto.  It's possible to make a keto diet that is compliant with almost any other restriction, such as vegan, vegetarian, Mediterranean, carnivore, and others.

Saturated fat, the estimated absolute risk and certainty of risk for mortality and major cancer and cardiometabolic outcomes: an overview of systematic reviews
Absolute risk is much more informative than relative risk- relative risk sounds more catchy but doesn't actually tell you as much.  When thinking about what a study means for you, whether its conclusion means you need to make changes, depends on the quality of evidence and not just the presence or absence of evidence.  This paper found that the evidence supporting a risk from consuming saturated fat was low or critically low quality.  When looking at the results of reducing or replacing saturated fat in terms of cancer mortality, the authors found that the evidence showed a range of 8 fewer deaths to 3 additional deaths per 1,000 people, and the certainty of evidence (quality) was low to very low.  So no clear-cut link between saturated fat consumption and risk of death from cancer.  When considering the impact of saturated fat in the diet and cardiac deaths, they found a rate of two deaths per 1,000 people with low to moderate certainty.  This does not support the strength of the message we get through doctors and public health to avoid saturated fat.

There are other factors to consider when seeing if the results of a study means anything for you individually.  To illustrate this point, consider one study that was included in this meta-analysis (with moderate quality evidence) that found 24 fewer deaths per 1,000 people from all-cause mortality in a treatment group that reduced saturated fat intake.  We need to look at the details to see what the implications are.  Most people being studied are eating the standard American diet, high in ultra-processed foods, and get their saturated fat from sandwiches, desserts and sweet snacks like cookies, cakes, ice cream, pastries; and rice and grain-based dishes like pasta and pizza, and milk and yogurt which is usually sweetened and flavored.  These foods are all high in sugars and simple starches.  Natural foods, including plain meats, cheeses, butter, etc are lower on the list.  The question then is how do you eat- if you eat lots of junk food, fast food, and processed food, then this study might apply to you.  If you already eat a diet primarily of whole foods and low processed foods then these results probably have no relevance for you.

"Nutrition science is full of low quality evidence that applies to a general population eating a low quality diet".  As a patient, we all deserve to have care providers who treat us as individuals.

Exploring New Ingredients for Cooking

These days many adults have grown up eating instant food such as canned soups, processed packaged foods such as rice mixes and seasoning packets, or if they have learned cooking "from scratch" it involves processed ingredients such as canned broth and pre-made dough.  It seems that not many people regularly cook from actual scratch and for many people this may be a very functional allocation of their time.  While this may (or may not) work well for some people it can make it difficult or expensive to try making dietary changes, as specialized processed foods may be prohibitively expensive, unpleasant to eat, or may be very hard or impossible to get.  Sometimes learning to make an ingredient from scratch (such as broth, mayonnaise, or salad dressing) is a good solution to this dilemma.  Another solution that can actually be quite fun is to learn to use ingredients that you have not worked with before and to learn to cook new types of dishes.  While this can be fun and exciting and very rewarding to do, it can take a lot of learning and adjustment as you get used to new ingredients and cooking methods.  Below is a collection of links that can help make this transition easier.  Or course what is new to one person is the comfort food that someone else grew up with, and will depend on where a person grew up, where their family and friends and neighbors are from, what restaurants and food stores they have had access to, their income, and many other factors.

Food Substitutions for Allergies and Special DIets

The following is a list of products and recipes for substitute foods to replace foods that need to be avoided due to allergies, intolerances, etc.  There has been a significant increase in the availability of such substitutes as allergies, food intolerances, and special diets have become much more common in the US in recent years, but many of these products don't taste good or don't work well for a variety of reasons.  So although there are a lot more options now it is still very helpful to have suggestions for which products are best.  The products and recipes listed below are ones that I have either tried myself and liked or have been liked and recommended by people I know.  Of course tastes are very individual so this is no guarantee that others will like them.  They are listed according to the food or ingredient that needs to be avoided.

NOTE- many of these foods contain ingredients that are problematic for many people, so it should go without saying to read the labels very carefully.  Inclusion on this list is not an endorsement of the food or ingredient, it is included so that people can decide for themselves what they want to avoid or not.

The Microbiome

There are about 10 times more microbial cells in and on our bodies than there are human cells.  These microbes are referred to as the microbiome, which is divided loosely into separate biomes based on location (such as the gut microbiome, the sinus microbiome, etc).  The way that these microbes affect and influence the health and functioning of our bodies has not been understood much in the past, but is increasingly being studied now.  The microbes that live in and on us play many roles, including aiding in digestion, providing certain enzymes and nutrients, regulating out immune system and inflammation, keeping pathological microbes in check, and even regulating the expression of some of our genes. New evidence shows that micro organisms are even the source of some of our genes which have been acquired through a process called horizontal transfer.

There are many factors that influence the health and diversity of our microbiome.  Many medications alter our flora, especially antibiotics, but many others do too such as oral contraception.  What we eat, what personal care products we use, and what we use to clean our homes are also major factors.  Antibiotics can have a devastating effect on the microbiome, something which is only now being appreciated, and which may explain many of the side effects and unintended outcomes of widespread antibiotic use.  A recent article about research being done on the role of the microbiome in autism and other health conditions called Autism's bacteria link gains credence provides a basic introduction to these factors:

"But changes in the past century have altered the microbial balance as societies relied too heavily on antibiotics, disinfectants, C-section deliveries and a diet of refined carbohydrates. Those changes replace inner gardens of helpful bacteria with a harmful mix that makes compounds that in excess can damage the digestive system, brain, immune system, and the way cells metabolize energy.  The damage is likely worse in babies whose brains and immune systems are developing, MacFabe says. But the damage also occurs throughout life."

The Origins of the Microbiome

A fetus growing in it's mother's uterus is sterile until birth, when it is colonized by bacteria.  In a vaginal birth, the baby is colonized by the mother's vaginal and fecal flora.  Babies born by c-section are instead colonized by the bacteria present in the room during birth.  Touching and kissing the baby is another source of flora.  To learn more about this read The infant gut microbiome: New studies on its origins and how it's knocked out of balance.  Once the baby is colonized at birth, the gut flora is further influenced by whether the baby is breastfed or formula fed and when solid food is started and what foods are eaten.

Maternal prenatal stress is associated with the infant intestinal microbiota.

"Results showed that maternal prenatal stress, i.e., either reported stress or elevated basal maternal salivary cortisol concentrations or both, was strongly and persistently associated with the infants' microbiota composition as determined by a phylogenetic microarray. Infants of mothers with high cumulative stress (i.e., high reported stress and high cortisol concentrations) during pregnancy had significantly higher relative abundances of Proteobacterial groups known to contain pathogens (related to Escherichia, Serratia, and Enterobacter), and lower relative abundances of lactic acid bacteria (i.e., Lactobacillus, Lactoccus, Aerococcus) and Bifidobacteria, altogether characteristics of a potentially increased level of inflammation. Furthermore, this aberrant colonization pattern was related to more maternally reported infant gastrointestinal symptoms and allergic reactions. In conclusion, clear links were found between maternal prenatal stress and the infant intestinal microbiota and health. Although causality cannot be concluded, the results suggest a possible mechanism by which maternal prenatal stress influences the offspring development. These results suggest a potential for bacterial interventions to enhance offspring health and development in pregnant women with stress."

How Diet Affects the Microbiome

What an individual eats has a profound impact on the population and diversity of their gut microbiota.  It is also true that there are differences between normal gut flora in groups of people living in vastly different parts of the world and eating vastly different diets.  A study was done comparing the gut flora of children in Europe with that of children in a rural village in Burkina Faso in west Africa that showed how significant these difference can be and suggests that our gut flora have evolved along with us to reflect what we eat.

Study links common food additives to Crohn's disease, colitis
The study being reported on here looked at two emulsifiers commonly used in processed food and their effect on gut health and gut flora, polysorbate 80 and carboxymethylcellulose.

"A key feature of inflammatory bowel diseases and metabolic syndrome is a change in the gut microbiota - the roughly 100 trillion bacteria that inhabit the intestinal tract - in ways that promote inflammation. In mice given emulsifiers, the bacteria were more apt to digest and infiltrate the dense mucus layer that lines and protects the intestines."

The Microbiome in Health and Disease

In this study Foxp3 T Cells Regulate Immunoglobulin A Selection and Facilitate Diversification of Bacterial Species Responsible for Immune Homeostasis (you can read about what this study means here) it is found that:

"They discovered that the immune system "sees" and responds differently to different bacterial communities. Rich and balanced bacterial communities seem to be perceived as "self" and induce a quick maturation of the immune system and gut responses (induction of regulatory T cells and IgA), while a poor and unbalanced bacterial community is apparently perceived as "non-self" and induces responses aimed at eliminating it (T cells with inflammatory properties and IgG or IgE responses)."

Understanding an individual's microbiome is part of the emerging paradigm shift going on in medicine right now to individualized medicine, which is an approach to understanding and treating disease based on an individual's genetics, microbiome, environmental factors, and any other factors that influence the health of an individual.  In this new paradigm, rather than classifying patients together by diagnosis and then treating each of them with the same methods it is a way to individualize treatment to the individual's specific needs.  This brief interview from a medical conference held in 2015 on the cutting edge research in digestive disease gives a little more detail on how this paradigm shift relates to the gut and it's microbiome.

Restoring Microbial Balance Key to Keeping Sinuses Healthy
"A new study by Dr. Goldberg and colleagues likewise concludes that the health of our sinus cavities, once thought to be largely sterile, may be highly dependent on the composition of their microbial residents...Beyond a significant decrease in the diversity of microorganisms among the patients, the researchers found a noticeable drop in a group of bacteria known as Lactobacilli, long associated with maintaining health in the gut. Concurrently, the researchers saw an increase in a little-known potential pathogen called Corynebacterium tuberculostearicum..."Central to the concept that we’re putting forward is that there is a protective mechanism in a normal sinus that comes about as a result of the microbiome,” Dr. Goldberg said."

The dormant blood microbiome in chronic, inflammatory diseases
"A number of recent, sequence-based and ultramicroscopic studies have uncovered an authentic blood microbiome in a number of non-communicable diseases. The chief origin of these microbes is the gut microbiome (especially when it shifts composition to a pathogenic state, known as 'dysbiosis'). Another source is microbes translocated from the oral cavity. 'Dysbiosis' is also used to describe translocation of cells into blood or other tissues. To avoid ambiguity, we here use the term 'atopobiosis' for microbes that appear in places other than their normal location. Atopobiosis may contribute to the dynamics of a variety of inflammatory diseases. Overall, it seems that many more chronic, non-communicable, inflammatory diseases may have a microbial component than are presently considered, and may be treatable using bactericidal antibiotics or vaccines."

Gut dysbiosis and detection of "live gut bacteria" in blood of Japanese patients with type 2 diabetes
"Mounting evidence indicates that the gut microbiota are an important modifier of obesity and diabetes...The counts of the Clostridium coccoides group, Atopobium cluster, and Prevotella (obligate anaerobes) were significantly lower, while the counts of total Lactobacillus (facultative anaerobes) were significantly higher in fecal samples of diabetic patients than in those of control subjects. Especially, the counts of Lactobacillus reuteri and Lactobacillus plantarum subgroups were significantly higher. Gut bacteria were detected in blood at a significantly higher rate in diabetic patients than in control subjects (28% vs. 4%, P < 0.01), and most of these bacteria were Gram-positive...The high rate of gut bacteria in the circulation suggests translocation of bacteria from the gut to the bloodstream."

The Treatment-Naive Microbiome in New-Onset Crohn’s Disease
"Inflammatory bowel diseases (IBDs), including Crohn’s disease (CD), are genetically linked to host pathways that implicate an underlying role for aberrant immune responses to intestinal microbiota. However, patterns of gut microbiome dysbiosis in IBD patients are inconsistent among published studies. Using samples from multiple gastrointestinal locations collected prior to treatment in new-onset cases, we studied the microbiome in the largest pediatric CD cohort to date. An axis defined by an increased abundance in bacteria which include Enterobacteriaceae, Pasteurellacaea, Veillonellaceae, and Fusobacteriaceae, and decreased abundance in Erysipelotrichales, Bacteroidales, and Clostridiales, correlates strongly with disease status. Microbiome comparison between CD patients with and without antibiotic exposure indicates that antibiotic use amplifies the microbial dysbiosis associated with CD. Comparing the microbial signatures between the ileum, the rectum, and fecal samples indicates that at this early stage of disease, assessing the rectal mucosal-associated microbiome offers unique potential for convenient and early diagnosis of CD."
It has recently been discovered that certain parasites are able to modulate the immune system of the host by communicating with the host's microbiome.  Some of these parasites are being used therapeutically to treat inflammatory and allergic disorders, although it was not fully understood why the treatment was effective.  Here findings are discussed that show that the parasites are inducing the microbes of the gut to produce certain short-chain fatty acids that modulate the immune system.

In addition, a compromised immune system can alter the microbiome of the gut, leading to intestinal disease such as Inflammatory Bowel Disease.

Implementing Dietary Changes with Children

The only thing more daunting for many of us than figuring out what dietary modifications a child needs, is figuring out how to get that child to eat this new way.  Many children with autism and related disorders have self-selected their diets down to only a handful of foods, often specific to brand, restaurant, or packaging.  The thought of offering this child broccoli or freshly juiced veggies sounds absurd.  But, like all things in the autism world these days, thousands of families have walked this road already and there is much wisdom, as well as many observations, tricks, methods, etc that can be learned from those that blazed the trail.  Here are a few that I have used or know others who have used:

When introducing a new version of a food- it's best to let some time pass since the child has had the previous version.  The more recently they have eaten the old version the more they may be aware of the difference in the new one.  Many people find it helpful to give the new food when the child is distracted, such as watching a video.  Feeding them may draw even less attention to the food, helping the child adapt to it.  The new food can be put into the packaging of the previously preferred kind.  For example I put the new homemade ketchup into the cleaned out bottle of the ketchup we had been buying.  Some parents have even gone to restaurants and gotten empty containers for chicken nuggets, fries, etc to put the new versions in.

Keep in mind that people's tastes change over time- very often children self-select to the foods that cause them problems.  These problems can include actually altering their sense of taste and/or texture, which can then reinforce the self-selection.  By removing these foods, you may be removing what is causing the avoidance of other foods.  What is surprising is how quickly this can change.  Many parents have been shocked by how quickly their child was willing and eager to eat new foods, that they could not imagine their child eating before, soon after the problem foods were removed.  Other times this can take longer, more like months, but I have seen this in action and it is amazing.  Also, our preferences for tastes, such as sweet and salty, is influenced by how much of this flavor we get....basically we adapt.  Someone who eats a lot of sugar will often need to eat a lot of sugar to taste something as sweet at all. By drastically cutting back on sweetness, in even a few days a person's sense of taste can be dramatically readjusted.

Preferences for food can be the result of underlying health problems- and may therefore be resolved with treatments, including dietary intervention.  A craving for sweet could be the result of a yeast overgrowth, and once the yeast is under control the child may not have nearly the same strong desire for sweet things.  A craving for salty foods may indicate an adrenal function problems.  Avoidance of meat is commonly associated with low levels of stomach acid, which in turn is often tied to low zinc levels.  Low zinc levels affect taste and smell in many ways and often results in a very restricted diet.  Supplementing zinc is often an easy and effective first step in dietary intervention with children.

Some miscellaneous thoughts- for some children, it is more successful to introduce new foods rather than new versions of familiar foods.  For other children, new versions of familiar foods are easier to adapt to.   Some ingredients can be "snuck" into the food, such as pureed vegetables, without drawing attention to the fact that there is anything new about the food.  Examples are putting pureed squash or sweet potato in pancakes or muffins, or putting bone marrow or ground organ meat in burger patties.  You may need to start by adding a very small amount so as not to change the flavor as much, then slowly increasing the amount.  Many kids prefer foods with a "carby" sort of texture, and may find meat hard to chew or just not a preferred texture.  One trick is to make chicken pancakes which are just chicken and eggs, pureed, and cooked up as pancakes.  These have the texture of "regular" pancakes but do taste like chicken.  This video of Julie Matthews and Polly Tommey shows how to make these pancakes.
